No-shows for SUS (Brazil's Unified Health System) appointments are a silent problem that drains public resources, demoralizes health teams and deprives citizens of care. In Brazil, an estimated 20% to 40% of appointments scheduled in primary care are not carried out - a rate that represents billions of reais in wasted installed capacity every year.
In a small municipality in the semi-arid region of Paraíba, that number was 30% before the DHF platform was implemented. Eighteen months later, it was 12.44%.
This article tells how that happened.
The context: a real municipality with a real problem
The municipality is in the interior of Paraíba. Like most small Brazilian municipalities, the Municipal Health Department operated with limited resources, a lean team and a scheduling system that depended almost entirely on in-person visits or phone calls.
The predictable result: queues before dawn, slots wasted by unannounced no-shows, and managers without reliable data to make decisions.
The 30% no-show rate was not just a bad number. It was a symptom of three structural problems:
- No active confirmation - patients booked and simply forgot, with no reminder and no easy way to cancel.
- No real-time data - the Department only knew how many no-shows had occurred after the day's schedule had already been wasted.
- Access barrier - rescheduling or canceling required going to the health facility in person, which made canceling more work than simply not showing up.
The implementation: what DHF put into operation
The DHF platform was implemented in the municipality in an operation that took less than two weeks to reach full operational capacity. The core components were:
Celina - virtual assistant via WhatsApp
Celina is the citizen's interface with the municipality's health system. Through the official WhatsApp Business API, any resident became able to:
- Request appointments and exams at any time, without going to the facility
- Receive automatic confirmations with the date, time and location of the visit
- Receive reminders 48 hours and 24 hours before the scheduled time
- Confirm, reschedule or cancel with a single text message
- Rate the care received right after the appointment
Choosing WhatsApp as the channel was no accident. With more than 90% penetration among Brazilian adults - including rural, older and low-income populations - the app removed the main digital exclusion barrier that makes conventional health solutions unworkable. Citizens did not need to download a new app or learn a new interface.
Management dashboard for the Department
Alongside the citizen channel, the Health Department gained access to a real-time management dashboard with:
- No-show rate by facility, specialty and period
- Real-time slot occupancy by UBS (basic health unit)
- Automatic daily summary of appointments completed, confirmed and canceled
- Satisfaction indicators collected through the scheduling channel itself
The results after 18 months
The following data were extracted from the platform's management dashboard and validated in quasi-experimental design studies.
| Indicator | Before DHF | After 18 months |
|---|---|---|
| No-show rate | 30% | 12.44% |
| Relative reduction | - | −50.2% |
| Citizen approval | - | 86% |
| Cancellations with advance notice | Close to zero | Significant |
The reduction from 30% to 12.44% represents a drop of more than 58 percentage points from the baseline - in other words, more than half of the patients who used to miss appointments without warning began to confirm or cancel in advance, freeing the slots for other citizens.
The 86% approval rate was obtained through a sample survey of citizens who use the system.
The municipality's Deputy Health Secretary assessed the impact plainly: after the system, foot traffic inside the Health Department was cut in half. Fewer queues, less administrative rework, more time for what matters.
What this result means at scale
The numbers demonstrate the model's viability for Brazilian municipalities - which make up the overwhelming majority of the country's 5,570 municipalities.
Reducing no-shows makes it possible to recover appointments that were previously wasted - without hiring a single new professional or building a single new UBS.
Lessons learned
Three factors were decisive for the result achieved:
1. The right channel for the right audience. WhatsApp works because it is where citizens already are. Solutions that require downloading an app or registering on a portal have much lower adoption rates, especially among rural and older populations.
2. Active confirmation, not passive. The automatic reminder changes behavior. Most no-shows are not intentional - they are forgetfulness. A reminder 24 hours ahead, with the option to cancel in one click, turns intention into action.
3. Data for the people who decide. A manager who does not know how many no-shows occurred in yesterday's afternoon shift cannot act to fix it. The real-time dashboard shifts management from reactive to proactive.
